By submitting this form, I acknowledge my intent to terminate my membership with the Arizona Dust Devils Baseball Organization.

Upon receipt of this form, the organization will review my account to determine if any fees are owed for the current month.

• If no outstanding fees are found, my membership will be canceled within 24–48 hours.

• If fees are due, the membership will be canceled following the next scheduled subscription payment.

I understand that all fees paid up to the player’s departure are non-refundable. Therefore, I will not request any financial compensation for voluntarily leaving the organization. *
